Express the dose in milligrams
per meter squared (mg/m2)or,
when applicable, milligrams per kilogram (mg/kg). e.g., for-
chemotherapy medications and
antibiotics for children.

A) All doses should be
calculated independently
(e.g., by the physician who
writes the order, the pharmacist
who prepares the dose, and the
nurse who administer it).

  B) Include the current weight or
body surface area with the
order.

For physicians : thank
you, you are calculating the
dose but it is always in your

 Welcome Message

Dear colleagues,
Dear colleagues,

Today's medicines are more complex and powerful than ever before. Increasing technology and number of patients these are the driving forces for growth in healthcare. Not only is the business of healthcare booming, but so are the increasing demand in higher levels in patient care.

When I started to work as clinical pharmacist with the hematology / oncology department I touched the sweetness of the teamwork spirit, a beautiful multidisciplinary team working together to provide the best patient care, I also believe that the pharmacist has a very important role in the patient care and education.

We are going to issue a pharmacy newsletter - The Bridge-about all issues related to the pharmacy. The Bridge will Contain cases from our daily practice and pieces of information.

I hope that The Bridge will connect colleagues from different areas sharing experience, knowledge and ideas, which will enhance our practice and attitudes.

The Bridge will not contain anyone name, as it is not to embarrass anyone.

I will gladly welcome any feedback or comments to enhance the teamwork and make our health care services in our country up to the standards.

Waiting for health provider pioneers kind feedback.
