Managing suspected cases of A (H1N1) Influenza presenting in the Airport
Stage One: Definition of suspected
case
Clinical criteria
Person MUST have one of the
following:
• Fever >38oC OR history of fever
AND flu-like illness (two or more of
the following symptoms: cough,
headache, rhinorrhea or vomiting/diarrhoea)
• Other severe/life-threatening
illness suggestive of an infectious
process
Epidemiological criteria
AND at least one of the
following in a person during the
seven days before onset of
illness:
• Close contact with a confirmed or
probable case of swine flu A (H1N1)
virus infection while the
case-contact was ill or in the 24
hours before illness onset
• T recently raveled (within last 7 days) to a geographical area known to have confirmed cases of swine influenza A/H1N1
Stage Two – Influenza Rapid Test
Positive for B influenza: A (H1N1) excluded. Treat accordingly.
Positive for A: Manage as a suspected A (H1N1) case and manage accordingly.
Stage Three:: Assess severity, treatment and disposal:
Mild febrile respiratory
illness with no clinical signs of
pneumonia: Start Tamiflu (osteltamivir)
75mg BD for 5 days and discharge
with written instructions. Transit
patients can continue with their
journey.
Moderate respiratory illness
with no signs of pneumonia but not
severe enough to be admitted to
hospital: Start Tamiflu and arrange
EMS transfer to Al-Gharafa centre
for further management.
Severe respiratory illness
or/and signs of pneumonia. Arrange
urgent EMS transfer to HGH

This is a nurse provided
screening service. Influenza Rapid
Testing is not performed at this
point of entry

testing facility or tamiflu supply.

the facility. Inform microbiology
and infection control. Provide the
appropriate level of care and
investigation under strict infection
control setting. Follow HMC policy
for airborne isolation precautions.
Managing probable cases of A (H1N1)
Influenza in Al- Gharafa centre
This centre was sanctioned by the
HMC as part of the preparedness for
the A (H1N1) influenza. It will act
as a hub for the continuing
management of selected influenza A
positive cases. It has access to CXR
facility.
The source of referral will be from
the screening points mentioned
earlier. Patients must be those who
are not suitable for home treatment.
It excludes children under the age
of 14.
This is a nurse and doctor lead unit
and will be suitably equipped to
accommodate as many as 15 patients
at a time.
It will provide observation period
for a maximum of 48 hours.
Patients who improve or / and remain
well will be discharged and will
complete the antiviral course
treatment. A written instruction
will be given.
Patients who show symptoms and signs
of disease progression that exceeds
the capabilities of the centre will
be transferred for hospital
treatment.
