DEPARTMENT OF OBSTETRICS AND GYNECOLOGY

The Department of Obstetrics and Gynecology offers a wide range of sophisticated services for complicated pregnancies, high-risk neonates, infertile patients and treatment of gynecologic problems. Some of these services were introduced early in the 1980s and later expanded, while others were evolved as the need arose.

In 1984, for instance, an Infertility Clinic was opened at the Outpatient Department to provide medical assistance to married couples unable to produce offspring. The Clinic provided consultations, hormonal treatment and microsurgery when needed.

Today, medical assistance to infertile patients includes a variety of sophisticated procedures in Assisted Reproduction Technologies (ART). The intra cytoplasmic sperm injection (ICSI) is a micro manipulation technique, which, aided by a powerful microscope, fertilizes the egg by injecting the sperm into it. Testicular Sperm Aspiration (TESA) and Microscopic Epididymal Sperm Aspiration (MESA) are techniques used in cases of obstructive azoospermia.

The micro manipulation technique has a success rate of 33.3%. Other methods in the ART program include the conventional IVF which has a success rate of 24%; Frozen Embryo Transfer (FET) with a success rate of 24.35%; GIFT technique with a success rate of 28.12%. All these services (including Intra Uterine Insemination - IUI) are offered at the Assisted Conception Unit (ACU) which was opened in April 1993.

The vaccination program first introduced in 1983 has been expanded in recent years. Beginning in 1990, newborns are now routinely vaccinated with B.C.G. and Hepatitis B vaccine, a drug administered to both mother and child. All primigravidae who are not Hepatitis B carriers receive Hepatitis B vaccine after delivery. All non immunized primigravidae are also vaccinated against Rubella before discharge from the hospital.

A Day Care surgery unit, set up in 1990, provides ambulatory surgical services. The unit provides minor gynecologic surgeries and circumcisions. In 1997, 4407 male circumcisions were performed while minor surgeries average 150 patients a year.

The Antenatal Care Program continues to be a coordinated activity between Primary Health Care Centers and HMC. Introduced in 1983, the program involves hospital consultants who visit health care centers on a rotating basis to provide a variety of post-natal medical care to mothers and their neonates.

The most recent program launched in the Department is the Cervical Cancer Screening Program that was started in May 1996. Already 2,608 patients were screened between May to October 1996. A total of 3,208 visits were made to this clinic in 1997. A "Well Woman Clinic" was established for the exclusive purpose of offering consultations for timely detection of cancer. Four women have showed invasive squamous cell carcinoma, 19 had low grade SIL and 16 had high grade SIL.
